Requesting a report from the Department of Justice
In order to process the request of your Department of Justice Report, or DOJ Report, follow the instructions below:
For California Residents
Begin by using the Live Scan form. This form can be printed out from the State of California Department of Justice website:

On the Live Scan form, mark “Record Review” on the “Type of Application” and mark “Record Review” on the “Reason for Application” line. Fill out the entire personal information section.
Once you have completed the Live Scan form, take the form to any local law enforcement agency where fingerprinting services are provided. If you would like to find a location nearest you, visit:

The fingerprint technician will guide you through the fingerprinting process and submit your DOJ report electronically. Typically, a DOJ Report takes approximately 1-2 weeks to be returned to the address you provide on the Live Scan form.
For Out of State Residents
If you are currently an Out of State Resident, you will need to manually submit a fingerprint card.
Begin by printing out the Application to Obtain a Copy of State Summary Criminal History Record which can be found on the DOJ website at: https://oag.ca.gov/sites/all/files/agweb/pdfs/fingerprints/forms/BCIA_8705.pdf
Once you have completed the Application form, you will need to go to a local law enforcement agency for fingerprinting services. On the fingerprint card, fill out your full date, date of birth, sex, and mailing address.
If you would like to obtain a fingerprint card to bring the agency, you may call the Record Review Unit at 916-227-3849 to request a fingerprint card to be sent to the address you provide.
The final step is to manually submit your Application form, fingerprint card, and processing fee (in the form of personal check, money order, or certified check) to:
Allow two to three weeks for your DOJ Report to arrive by this method.
