To obtain a copy of the records of your Nevada Criminal History, you must be the subject requesting the records. This means that you alone can request your own criminal records.

However, if you are seeking only your Nevada Criminal History Record, you will need to begin by completing the Identification File Request for Nevada Records of Criminal History Form, DPS-006 (PID). This form can be printed out from the Nevada Repository website at: https://www.nvrepository.state.nv.us/Criminal/forms/DPS_006_Form.pdf
Once you have filled out the DPS form, you will need to obtain proof of identity. Proof of identity needs to be submitted in the form of one original fingerprint card which will include personal information such as your name, date of birth, and place of birth.
A fingerprint technician must complete your fingerprint card. You can find a fingerprint technician at any local law enforcement agency. Your signature will be required on the DPS form, as well as flat impressions of all ten fingers.
Along with the DPS form, you will need to include a $21 money order or certified check made payable to the Department of Public Safety.
